Arkham Origins – 18 – Final Boss – Bane

Just like the first meeting, Bane’s charging rush cannot be countered, simply evade out of the way using the left stick + A. Once he jumps up, evade away, run up near him, then Super Cape Stun by pressing LT 3x . Beat him up, then rinse and repeat the whole process. When the thugs enter, simply evade away from everyone using left stick + A again, as Bane himself will eventually knock out all of this allies. Follow this video for reference:

For the second form, just keep hiding at all costs and only attack from behind using a stealth approach. The best way to damage him quickly is place yourself underneath a grate, making sure you're located at the middle of the grates, not at the ends. Spring out fast when you get the takedown prompt to damage him. Damaging him nearby an electric cabinet is of utmost importance, as it will deal much more damage and stun him good. Just use the disruptor if in any case your system will be jammed, and also use the glue grenades to hold him back if he ever finds you.

For the last sequences, simply follow the prompts and finish the game. It is simply a QTE that is very straightforward and generic. Enjoy the ending.
